This position will provide surveillance, consultation, guidance, audits and evaluation of health care providers to assure a level of quality care based upon compliance with Federal and State laws and rules. This position will also provide training and expertise to health care professionals to educate them on the MDS Assessment standards to ensure compliance.
Working as a Health Facility Evaluator, you will:
+ Evaluate and conduct audits on providers to ensure a level of compliance with State and Federal laws and rules and to audit the accuracy of coding MDS assessment forms and the resulting Case Mix classifications and to provide training, as allowed, to providers to prevent violations as related to applicable regulations and standards of practice.
+ Make and conduct audits on providers to ensure a level of compliance with State and Federal laws and rules and to audit the accuracy of coding MDS assessment forms and the resulting Case Mix classifications and to provide training, as allowed, to providers to prevent violations as related to applicable regulations and standards of practice.
+ Participate in continuing education program to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.
+ Attend legal proceedings regarding correction orders, issuance of penalty assessments, so that all matters and concerns therein will be properly presented and clarified.
**Position will be based out of the Fergus Falls office location with up to 75% of work time spent onsite performing surveillance of facilities predominately located in the surrounding southeastern region of Minnesota.**
**Occasional travel outside of the base region is expected, to survey facilities located throughout the state of Minnesota. When not onsite, remaining follow-up work is performed by teleworking from your home office.**
**Most of work hours are between 8:00am - 4:30pm, Monday - Friday but candidates must be available for occasional weekend work and overnight travel. The need for weekend, overnight, or travel outside of the base region is determined by audit workload.**
**Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.**
**Minimum Qualifications**

Two (2) years of experience in a health care setting in positions that may include the following:

Currently licensed in Minnesota as a Registered Nurse, without restriction.

Working as a Rapid Response Evaluator, you will:
+ Evaluate providers' compliance with State and Federal laws and rules and to investigate allegations of non-compliance and/or maltreatment.
+ Compile and record the investigation information which supports conclusions reached and documents the degree of compliance with state and federal statutes and rules.
+ Make unannounced follow-up site visits to ascertain compliance with correction orders previously issued.
+ Attend legal proceedings and testify regarding substantiated complaint cases, uncorrected orders/deficiencies and other matters as required to assure that the department's finding/position is properly presented, clarified and supported.
+ Participate in continuing education program to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.
**Positions will be based out of the primary office location with up to 50% of work time spent onsite performing surveillance of facilities. When not onsite, remaining follow-up work is performed by teleworking from your home office.**

Working as a Rapid Response Evaluator, you will:
+ Evaluate providers' compliance with State and Federal laws and rules and to investigate allegations of non-compliance and/or maltreatment.
+ Compile and record the investigation information which supports conclusions reached and documents the degree of compliance with state and federal statutes and rules.
+ Make unannounced follow-up site visits to ascertain compliance with correction orders previously issued.
+ Attend legal proceedings and testify regarding substantiated complaint cases, uncorrected orders/deficiencies and other matters as required to assure that the department's finding/position is properly presented, clarified and supported.
+ Participate in continuing education program to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.
**Positions will be based out of the primary office location with up to 50% of work time spent onsite performing surveillance of facilities. When not onsite, remaining follow-up work is performed by teleworking from your home office.**

This position serves to protect vulnerable individuals receiving care in facilities licensed and/or certified by the Minnesota Department of Health through investigating allegations of compliance violations and/or of maltreatment. To apply nursing and health related knowledge to evaluate data obtained, interviews conducted, and observations completed during an investigation. Documenting the investigation and statements of deficiencies as it relates to compliance and maltreatment.
Working as a Rapid Response Evaluator, you will:
+ Evaluate providers' compliance with State and Federal laws and rules and to investigate allegations of non-compliance and/or maltreatment.
+ Compile and record the investigation information which supports conclusions reached and documents the degree of compliance with state and federal statutes and rules.
+ Make unannounced follow-up site visits to ascertain compliance with correction orders previously issued.
+ Attend legal proceedings and testify regarding substantiated complaint cases, uncorrected orders/deficiencies and other matters as required to assure that the department's finding/position is properly presented, clarified and supported.
+ Participate in continuing education program to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.
**Positions will be based out of the primary office location with up to 50% of work time spent onsite performing surveillance of facilities. When not onsite, remaining follow-up work is performed by teleworking from your home office.**
**Position location will be determined by the top candidate and will be based out one of the following MDH district office locations: St. Paul, St. Cloud, Duluth, Rochester, Marshall, Bemidji, Fergus Falls.**
**The hours of work are generally between 8:00 a.m. - 4:30 p.m., Monday - Friday, and the need for weekend or overnight travel is determined by survey workload.**
**Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.**
**Minimum Qualifications**

Two (2) years of experience in a health care setting in positions that may include the following work experience:

Current MN RN License
A degree in Nursing, Public Health, Health Care Administration, Psychology, Social Work, in the medical field or closely related area may substitute for experience as follows:
+ Doctorate degree: 24 months
+ Master's degree: 18 months
+ Bachelor's degree: 12 months
+ Associate degree: 6 months
**Additional requirements to be assessed during the interview process:**
+ Oral communications skills sufficient to interview staff, residents, clients and patients.
+ Critical thinking skills sufficient to collect and analyze information, identify areas of focus, and make decisions based on evidence gathered.
+ Listening skills sufficient to ascertain level of compliance in meeting resident/client needs.
+ Interpersonal skills to develop and maintain effective working relationships, inform providers of deficiencies and other pertinent findings, and participate in legal proceedings.
+ Written communication skills sufficient to document concise, accurate and complete information and conclusions.
+ Computer skills sufficient to document findings and conclusions.
**Preferred Qualifications**
+ Clinical and/or management experience in assisted living facilities and/or home care.
+ Experience conducting regulatory, licensing, and/or healthcare-related investigations.
+ Experience working with diverse backgrounds, religions, or gender identities.
+ Ability to speak multiple languages; bilingual.
+ Ability to apply nursing and health knowledge and clinical expertise to the review of providers.
